Cancellation Policy
We ask that you give 24-hours’ notice if you need to cancel your appointment. If this is not possible due to an emergency, please contact us at the spa as soon as you know that you cannot make your appointment. The first missed appointment is a “learning experience,” any further missed appointments without 24-hours’ notice will result in being charged a fee for your appointment. You will also be required to provide a deposit when setting appointments. If a gift certificate is being used as payment for your appointment, and you fail to give a FULL 24 hour advance cancellation notice, that gift certificate will become null and void.
Liability Waiver Text
You must provide complete and accurate health information and notice of health changes at successive appointments as appropriate. Massage therapy should not be performed under certain circumstances, you must inform the therapist of any changes in your health history and you release the massage therapist from any liability if you fail to do so. Understand that massage therapy is designed to be an ancillary health aid and is not suitable for primary medical treatment. The massage therapist does not diagnose illness or disease, perform spinal manipulations, prescribe medical treatments, and nothing said or done during the session should be construed as such. Massage therapy will be given as agreed upon by therapist and client for the predetermined goals of stress reduction, relief of muscular discomfort, and/or health promotion. You must immediately inform the massage practitioner of any unusual sensation or discomfort so that the pressure or methods may be adjusted to your level of comfort. You must understand that the massage is not sexually oriented in any way and that any illicit or suggestive remarks or behavior on your part will result in immediate termination of the session and full payment will be required. The breast and genital area will not be massaged under any circumstances. Permission will be asked before working close to these areas; otherwise, a professional distance will be maintained. Understand that a single massage session or massage used on a random basis is limited to providing a general, nonspecific approach using standard massage methods and does not include any methods to address soft tissue structure or function specifically. Continuous consecutive appointments are required for proper therapy treatments. We do not massage anyone under the age of 18 without signed parental consent and direct supervision. The discussion between the massage therapist and the client is confidential. The client may or may not choose to talk during the massage. Any health information disclosed, observed or treated will be kept confidential under HIPAA law.
Purchase Policy
Membership Purchases: Membership plans are offered in increments of 3 months, 6 months, 9 months, or 12 months. Unredeemed pre-paid massages will roll over to the next month. Memberships are non-transferable. Massage sessions purchased under membership may combined to multiple treatments but cannot be split (ex: NO:one hour massage split into two 30 minute massages YES: 2 credits/months of massages for a 2 hour session.) Membership plan rates may increase at any time, but your rate is locked in for the life of your membership. At client request, plans may be deferred twice during the lifetime of the membership. Deferment may be up to 6 months. For military it will be based on orders. Additional discounts and promotions may not be combined with your reduced member rate. Membership plans are provided at a significantly reduced rate and are therefore are non-transferable and may not be shared. Client no-shows or late cancellations (less than 24 hours prior to appointment) will be billed at the member rate. Gift Certificates: Gift certificates are transferable. (From client to client) Gift certificates are non-refundable. If purchased at full price, your gift certificate does not expire. If purchased at a discount, your gift certificate is valid until the expriation date noted on the certificate, after which your certificate is valued at the original price paid. (Ex: A $100 gift certificate purchased for $75 will be valued at $75 after expiration.) Packages: Packages are non-transferable and non-refundable. Massages purchased in a package may not be divided into multiple treatments (ex: a one hour massage split into two 30 minute massages.) In the event of unforseen circumstances (sudden move or illness) which prohibits the use of part or all of your package, contact us to discuss options. The services of a massage therapist or bodywork professional may be covered by health insurance when prescribed by a chiropractor or osteopath. If this seems like an option for you you will be provided with a medical release form and a physicians referral.
Terms and Conditions
Please be available for your appointment 10 minutes prior to the scheduled start time. Allow time for a consultation and for you to complete any new client forms. If late arrival is inevitable, your service may be shortened in order to keep on schedule. The original treatment time will be set and you will still be charged the full amount for the session. Unfortunately we cannot always provide late arrivals with an extension of scheduled appointments. In special cases we may be able to accommodate a partial or full appointment. This will be at our discretion and only with proper, advanced notification of your late arrival. The full session price will be charged. Upon finding that a client is absent from their appointment, the therapist will attempt to contact the client, and will remain at the appointment location for 15 minutes. After this time the appointment will be considered a "no show" and the client will be charged the full fee for the service scheduled and will be mandatory to pre-pay for future services.
Return/ Refund Text
Memberships, gift certificates, and packages are not refundable. There will be no refunds for services completed. Employees have the right to refuse service for just reasons. Refunds will be under the discretion of the owner/manager/sole therapist at that time. Under few circumstances will there be appointments that's are not booked ahead of time due to this software. Which requires payment upon booking and 24 hour notice of cancelation for full payment returned. All services will be paid before services are given.

Third Party Service Providers: We may disclose information about individual users to third parties who provide services to Toni's Oasis LLC / Oasis Mind Body and Soul. These third parties provide a variety of services to Toni's Oasis LLC / Oasis Mind Body and Soul, including, without limitation, verifying credit card information, tracking Internet marketing campaigns, and processing credit card transactions. In addition, these services may include enhancing the information Toni's Oasis LLC / Oasis Mind Body and Soul collects about its customers. We may also use third-party email providers to assist with sending our marketing emails. These companies do not retain, share, store or use personally identifiable information for any secondary purposes. Any personally identifiable information that Toni's Oasis LLC / Oasis Mind Body and Soul shares with its service providers may only be used to provide services to Toni's Oasis LLC / Oasis Mind Body and Soul and the service providers will not be given permission to use such information for other purposes. If we decide to change our privacy policy, we will post those changes to this privacy statement, the homepage, and other places we deem appropriate so that you are aware of what information we collect, how we use it, and under what circumstances, if any, we disclose it. We reserve the right to modify this privacy statement at any time, so please review it frequently.
